Our Registered Dietitians assess your specific needs and
help you develop a nutrition plan that meets your goals and fits into
your budget and your lifestyle. Individual or group sessions consist
of Medical Nutrition Therapy (MNT) approaches that integrate nutrition
and lifestyle to prevent and improve illness.

Workshops
"Balancing Act Diabetes Management"
- Education is provided in the form of group instruction
and individual sessions with our Registered Dietitian.
- Learn the basic "survival skills" necessary
for home self-management of diabetes and how to fine tune them into
problem solving and lifelong habits.
- Receive a blood glucose monitor and learn how
and when to use it.
Programs are age specific, family based and culturally
sensitive with services provided in English and Spanish.
"Healthy Choice Nutrition &
Fitness"
- Adults 18 years & older
- Group education and individual sessions with a Registered
Dietitian provide a non-diet approach to weight management.
KinderShape® & KidShape®
Group sessions focus on healthy eating, physical activity and healthy
lifestyles for the entire family. Sessions are conducted by a team of
professionals led by a Registered Dietitian. Classes are held once a
week and are 2 hours long. A parent or guardian is required to attend
with each enrolled child; however, all family members are encouraged
to attend.

About Our Staff:
All of our programs are led by a Registered Dietitian and may also include
a mental health professional and exercise specialist. Programs and services
are offered in English and Spanish.
Putting it all together:
Our Registered Dietitians help you create a wellness plan that includes
your personal preferences and fits into your lifestyle. Good nutrition
habits have been shown to promote a feeling of well being and personal
empowerment, prevent and treat illness, and reduce health care costs.
A complete nutrition assessment
will include:
- Medical and social history
- Nutritional history and analysis
- Review of diagnostic tests, medical treatments and
medications
- Lifestyle evaluation
Program Information:
There is a fee for services. However, many insurance plans will cover
the cost. Scholarships to attend classes may be available.
How to refer a patient:
- Provide the patient with a written referral along
with Diagnosis and ICD-9 code.
- Follow insurance plan guidelines for authorization
if required by plan.
