WIC - First Visit

Here’s what you will need on your first WIC visit

  • Proof of income.
    • Paycheck- current paycheck
    • Tax return (up to April)
    • Letter from employer
    • Unemployment benefit statement
  • Proof of residence identity/address. One of the following:
    • Utility bill
    • Mail that has gone through the postal system
  • Identification – driver’s license, passport, etc
  • Individuals with proof of TANF or Food Stamps meet WIC income eligibility.
  • WIC referral form completed by Health Care Provider
  • The child/infant or woman enrolling must be present.

What to expect at your first visit

During your first visit you will meet with an individual counselor to determine nutritional risk. This will include a height and weight check and a series of questions regarding your food intake.

If you are found to be eligible for WIC benefits, you may receive vouchers that day to purchase nutritious foods.
